Insurance rates are set by your carrier and dependent on individual copays and deductibles.  We do accept most major insurance carriers.  At this time however, patients will need to pay for their appointment on the day they are seen.  Patients will be able to pay a discounted rate and their personalized claim form will be provided after each visit. Patients will be responsible for mailing in their claim forms for direct reimbursement by their company.
